LiveOnNY is currently looking for Family Support Advocate – Fulltime
Do you possess the attributes to be a successful Family Support Advocate and perform the following essential functions?
Responsibilities
The Donation Advocate provides onsite response, is responsible for advocating for donation and supports families through the organ donation process. Initiates timely donation conversation with families either in person or telephonically and in collaboration with hospital partners. Maintains authorization rates that are equivalent or exceeds the national average. Supports and fosters the Mission, Vision and Core Values of LiveOnNY and its commitment to organ donation.
**This role requires frequent travel (valid driver license and own vehicle) and ability to commute to different hospital partners within our service area**
- Donation Advocate Responsibilities:
- Delivers high-quality support to potential donor families.
- Facilitates the opportunity for donation by keeping hospital staff informed regarding the plans and progression of the referral process.
- Offers donation in an empowering and inspiring manner. Responsible for educating potential donor’s Next-of-Kin (NOK) about donation options, status of donor designation, or requesting authorization for donation from legal NOK.
- Obtains authorization or disclosure with first person authorization and conducts a uniform donor risk assessment questionnaire (UDRAI) with appropriate NOK/surrogate of the potential organ donor.
- Assess families understanding of grave prognosis and brain death declaration.
- Supports donor families by attending OR in Donation After Circulatory Death Donation (DCDD).Completes accurate, and timely documentation of donor-related activities in computerized confidential donor record, and forms according to established policies and procedures.
- Works in strong collaboration with hospital personnel, LiveOnNY’s Family Service Lead (FS Lead), LiveOnNY’s Administrator-on-Duty (AOD), Referral Administrator (RA), and clinical staff to evaluate and determine the best time to discuss the opportunity for donation with potential donor families.
- Completes accurate, and timely documentation of donor-related activities in computerized confidential donor record, and forms according to established policies and procedures.
Qualifications
A License Master’s Degree in Social Work, is required. Excellent knowledge of the dynamics of grief and bereavement.
1-2 years’ experience with grief counseling, psychology or social services, crisis intervention, with proven track record of experience.
Experience in trauma/critical care setting helpful.

Salary Range: $75,000-$85,000
